If your Dog is Spayed or Neutered please upload one of the following documents (the notation on a rabies certificate is not valid as proof):

1. Spay/neuter certificate

2. Surgical documents that clearly identify your pet and the paid procedure

3. Letter from a veterinarian stating they have examined the pet and it has been spayed or neutered.


Please provide the "rabies certificate" the invoice is not an acceptable document. The certificate should identify the type of rabies vaccination including lot #, manufacturer, etc.
